Job Summary
Care Partner with HammondCare delivers client-centred care planning and clinical care management to support older adults living at home. The role collaborates with clients, families and multidisciplinary teams to develop and implement personalised care plans, manages care delivery within funding and service expectations, and supports the Client Care Operations Manager to grow the client base. Requires formal qualifications in aged care/disability/community services (Certificate IV or Diploma level) and a minimum of two years’ experience in aged care or home/community care, excellent communication and relationship-building skills, knowledge of Home Care Packages Program and Aged Care Quality Standards, strong organisational and coordination abilities, and a valid driver’s licence with willingness to travel between HammondCare sites (St Leonards/Miranda). Required Qualifications
- Certificate IV in Aged Care
- Certificate IV in Disability
- Certificate IV in Community Services
- Diploma of Community Services (Case Management)
- Diploma of Ageing Studies and Services
- 2 years experience in aged care, home care, or community care
- Strong communication and relationship-building skills
- Knowledge of Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme
- .Driver's licence and safe driving record
- Willingness to travel between HammondCare sites and services
- Experience operating in a multi-site environment
