Job Summary
As a Care Partner, you will guide customers through their care journey, focusing on developing holistic health plans and coordinating multidisciplinary services. Responsibilities include conducting onboarding assessments, collaborating with families and clinical teams, supporting care transitions, maintaining accurate records, and identifying opportunities for additional services. Required skills include a strong customer focus, excellent communication skills, and the ability to handle sensitive conversations with empathy. Candidates with previous experience in aged care and relevant qualifications are preferred.
