Job Summary
Care Partner (Clinical) – Registered Nurse for Aveo Care at Home. Manage a caseload (~45) of residents in their own homes across Ormiston and Cleveland, Queensland. Develop personalised care plans, coordinate services, provide tailored information on care services and funding options, maintain records and reports, and deliver high-quality customer service to residents, families, staff, and external health professionals. Engage with external service providers to deliver home care packages and support throughout the My Aged Care journey; promote Aveo Care at Home services through community events and care package transfers. Requires current RN (AHPRA) registration, home/community care experience, understanding of the Support at Home program, strong communication and tech skills, and a valid driver’s license with two COVID vaccinations. This role offers a permanent full-time schedule, an Aveo vehicle and integrated tools, and opportunities for professional growth within a resident-facing environment.
Required Qualifications
- Current Registered Nurse AHPRA registration
- Drivers License
- Two COVID Vaccinations (required)
- Prior experience in the home care or community care industry (e.g. Case Manager, Care Coordinator and/or exposure to My Aged Care or ACAT)
- Solid understanding of the Support at Home program (or previous CHSP or HCP knowledge)
- Excellent customer service, technology and communication skills
