Job Summary
Care Partner role focused on guiding customers through their care journey with compassion, developing holistic health plans, coordinating multidisciplinary services, and delivering safe, person-centred care to enable aging well at home or in a chosen setting. Responsibilities include onboarding assessments, health plan development, coordinating clinical and external providers, supporting care transitions and discharge planning, maintaining accurate records and contributing to audits and quality improvement, identifying opportunities for additional services, promoting safety and compliance, and collaborating within a team to achieve optimal outcomes. Requires experience in community/aged care, strong customer focus, excellent communication, and knowledge of funding streams and quality standards. Desirable qualifications include Certificate IV or Diploma in Community or Aged Care. Hybrid work model with Cairns office presence 3 days per week.
