Job Summary
Care Partner role providing telephone-based and telehealth client care for elderly Australians in Clayton, Victoria, with a hybrid work model between the Clayton office and home. Responsibilities include care planning, assessment and coordination, developing and reassessing care plans with clients and families, coordinating with Nurses and other Managers, monitoring care-package budgets, facilitating client onboarding, and ensuring compliance with relevant standards. Requires strong communication skills, CRM and Microsoft Office proficiency, ability to manage multiple clients and stakeholders, and an right to work in Australia with National Police Clearance. Full-time, permanent role with a telehealth-first care model and 9–5 weekdays.
Required Qualifications

- Excellent verbal and written communication skills
- Strong computer skills in CRM systems and Microsoft Office
- Ability to work at pace with multiple clients and stakeholders
- Experience in aged care (desirable)
