Care Partner

Care Partner in Anglicare Sydney (Australia) Full-time role based in Macquarie Park, supporting home care clients across Northern Sydney and Northern Beaches. You will assess needs, match services and assistive technology to client budgets, ensure compliance with Aged Care standards and programs, manage finances and documentation, and build positive relationships with ACAT teams, health professionals and brokered providers to enhance wellbeing and independence. Requires empathetic, organized collaboration with strong case management skills and a commitment to integrity, justice, compassion and excellence. Qualifications include a Certificate IV in Aged Care/Community Services or equivalent. Desirable Diploma in Case Management and experience in client assessment and budget management.
