Job Summary
Coordinate individualized supports and services for clients living with dementia from onboarding through to review, ensuring continuity of care across Shenton Park. Conduct comprehensive client assessments, develop care plans, and champion person-centred care while maintaining financial stewardship and program accountability. Monitor client wellbeing through regular check-ins, ensure compliance with Aged Care Quality Standards, and provide clear information regarding service options, costs, and rights. Build relationships with local referrers, including My Aged Care and assessment teams, to grow and manage the client portfolio. This Registered Nurse role supports meaningful change in dementia care within a purpose-driven team offering competitive salary packaging and ongoing professional development.
Required Qualifications
- Registered Nurse (RN) with current AHPRA registration
- Demonstrated experience in service coordination within aged care, disability, or community services
- In-depth understanding of Support at Home (SAH) services
- Proven ability to assess client needs and develop appropriate care plans
- Strong organisational skills, with the ability to plan, prioritise, and meet deadlines
- Empathy and experience supporting people living with dementia
- Excellent verbal and written communication skills
- Ability to build effective relationships with clients, families, and internal stakeholders
- Demonstrated commitment to positive client outcomes, with integrity and professionalism
- Ability to work both independently and collaboratively within a team
- Working rights in Australia
- Current National Police Clearance
- Valid WA 'C' Class Driver's Licence and access to a reliable vehicle
Desired Qualifications
- Certificate IV in Aged Care, Community Services, or a related qualification
