Job Summary
Lead and oversee client and caregiver operations in a home care setting. Responsibilities include completing new client visits and facilitating care plan completion, following up with clients, resolving concerns, and coordinating post-hospital check-ins. Manage caregiver engagement and retention, coordinate ongoing training, and conduct regular caregiver check-ins. Support Medicaid onboarding and authorizations, maintain accurate records, and provide scheduling and general office support as needed. Requires strong communication, problem-solving abilities, organization, and reliable transportation. This is a stable, full-time leadership role focused on ensuring high-quality care for clients and supportive supervision for caregivers.
