Job Summary
Conduct health screenings and intake appointments via phone, building rapport to gather accurate information on health, food, transportation, and housing. Schedule follow-up appointments directly in Healthie, ensuring consistent patient relationships and high show rates. Connect patients to appropriate resources, such as doctors or Community Health Workers, after completing intakes. Enter patient data into records systems in real time, reviewing notes for accuracy and adhering to HIPAA privacy rules. Manage daily targets including a 60% show rate while working remotely with limited supervision.
Required Qualifications
- High school diploma or equivalent
- Fluency in English and Spanish (or another language)
- Score 80% or higher on the final training assessment
- Pass a drug test and background check
- Ability to sit, stand, and walk regularly
- Good hearing and vision
- Ability to use a computer to enter and update health information
- Ability to keep a reliable internet connection and a quiet, private space for remote calls
- Ability to manage daily schedule and consistently hit targets including a show rate of 60% or higher
- Ability to space out appointments to review notes carefully
- Ability to follow all privacy rules (HIPAA) and get formal patient consent for every call
- Ability to enter patient information into records system in real time
- Ability to review notes for accuracy and finish all documentation before the end of a shift
- Ability to build quick rapport with patients
- Ability to ask open-ended questions to understand patient needs
- Ability to schedule the right next step for patients
- Ability to book intake appointments directly in Healthie
- Ability to follow up to confirm appointments

Desired Qualifications
- Degree in Human Services, Public Health, or Behavioral Health
