Job Summary
Support families and individuals in accessing State Plan Medicaid and HCBS services by educating them on independent living options and PASCO's assistance. Listen to client concerns, provide helpful resources, and clearly communicate why PASCO is the best choice for long-term home health. Conduct required assessments to support families while acting as the first point of contact for the community. Use Salesforce and data systems to document details, schedule internal staff for evaluations, and coordinate onboarding for caseload prospects. Ensure PASCO remains compliant with Medicaid requirements and grows the number of families accessing services.
Required Qualifications
- High School diploma or equivalent
- Moderate or exceeding ability with Microsoft Office products, specifically Outlook
- Strong computer skills including Microsoft Office Suite, Outlook
Desired Qualifications
- Experience with CRM/Salesforce
- Experience with a healthcare documentation system
- Experience with Medicaid or Home Healthcare experience
- Familiarity with electronic health record systems
- Energetic, well-spoken and the ability to connect with people on the phone
- Strong listening skills and the ability to ask targeted questions
- Ability to multi-task, prioritize, and manage time effectively
- Ability to work quickly, efficiently and with a high degree of accuracy
- Persuasive and goal oriented
- Self-motivated and self-directed
- Strong teamwork and relationship building skills
- Excellent organization and time management skills, the ability to maintain one's own schedule, and meet deadlines
- Proven skills in customer satisfaction
