Care Navigator (OR - Coos Bay/Coos County)

Job Summary

Meet with members in their homes to complete preventative screenings and perform in-home preventative care testing and education. Collaborate with physician practices to review medical charts for care gap closures, communicate with patients to obtain current statuses and provide education, and coordinate appointments. Collect and submit data to and from the EMR while maintaining patient confidentiality. Travel to assigned members' homes within a two-hour radius of Coos Bay, adhering to outpatient treatment center policies and procedures.

Required Qualifications

  • One (1) year of medical experience
  • RMA, CMA, CNA, and/or EMT experience
  • Must have the willingness to drive with a clean driving record
  • Reliable transportation
  • Excellent communication skills, both verbal and written
  • Requires problem-solving and decision-making
  • Must be able to work in a self-directed environment
  • Identify and resolve problems in efficient and effective ways
  • Takes personal responsibility for the quality and timeliness of work and achieves results with little oversight
  • Adapts to changing business needs, conditions, and work responsibilities
  • High School Diploma or GED required
  • CPR/BLS certification
  • Annual TB testing
  • Unrestricted driver's license
  • Must be able to frequently drive long distances, which may require a two-hour radius of your designated county
  • Must be able to physically perform the essential duties of the position which include lifting 30 lbs.

  • Must be able to physically perform the essential duties of the position which include hearing
  • Must be able to physically perform the essential duties of the position which include typing
  • Must be able to frequently drive long distances, which may require a two-hour radius of your designated county (travel time is paid and mileage reimbursement is available)
  • Occasionally required to stand
  • Occasionally required to walk
  • Continually required to sit
  • Occasionally required to climb, balance, bend, stoop, kneel, or crawl
  • Continually required to talk or hear
  • Must be comfortable with working in a variety of conditions, including members' homes, physicians' offices, cold conditions, and hot conditions
  • Must be able to lift and/or move more than 30 pounds

Desired Qualifications

  • Associate's degree is preferred
  • Medical Assistant schooling is strongly preferred
  • Medical Assistant certification preferred
  • Certified Nursing Assistant certification preferred
  • Emergency Medical Technician certification preferred
