Job Summary
Care Navigator in-field and telephonic support for the In-Patient Case Management team; serves as a liaison between patients, providers, hospitals, and case managers to ensure continuity of care, timely coordination of services, and positive patient outcomes. Responsibilities include developing care plans and transitional arrangements, listening to patient concerns, identifying and documenting care coordination barriers, conducting patient outreach and follow-up to assess status and reinforce treatment plans, ensuring treatment requirements are met, coordinating appointments and referrals, and maintaining timely documentation. The role supports field-based work with hybrid remote options, requires travel to partner hospitals, and targets compensation in the $18-$25/hour range (annualized to $37,440–$52,000 depending on location and experience). The candidate should be proficient with Healthcare Management Systems, MS Word/Excel, and communicate effectively in bilingual settings where applicable.
Required Qualifications
- 1+ years of Doctor’s office/clinic setting, CHW, MA, or CNA experience
- Proficient with Healthcare Management Systems and Microsoft Office Word and Excel
- Bilingual: Spanish/English, Mandarin/English, or another second language is strongly preferred
- Outstanding communication skills, both written and verbal
- Excellent organizational skills
- Ability to travel locally (visiting hospitals)
- Strong comfort with working in a fast-paced environment
