Care Navigator (Inbound)

Job Summary
Inbound Care Navigator role supporting a team of clinicians by providing telephonic coordination of care for Upperline+ patients and families; scheduling in-home, clinic, and telehealth visits; coordinating referrals and orders to completion; coordinating preventive care needs; collaborating with an interdisciplinary team to deliver complex care plans; requires associates degree or higher and prior healthcare experience; ideal candidates have patient-engagement experience, strong customer service and organizational skills, and the ability to work independently in a virtual setting. Remote opportunity available to residents of AL, AZ, FL, GA, KY, IN, IL, MI, PA, SC, and TN.
Required Qualifications
- Associates degree or higher education required
- Prior healthcare experience, ideally serving chronically ill patients, required
- Patient engagement experience highly preferred
- Strong customer-service orientation
- Exceptional phone etiquette
- Exceptional organizational skills and ability to multi-task
- Self-starter with an entrepreneurial spirit and demonstrated problem solving skills
- Strong writing, Word, Excel, and database management skills
- Access to a strong & secure internet connection
- Ability to work independently in a virtual setting
- Dedicated private workspace
