Job Summary
Care Navigator role supporting Charlie Health’s admissions team by driving the admissions process for youth, coordinating outbound referrals, and ensuring a seamless, compassionate experience for clients and families. Responsibilities include working directly with clients and referral sources to understand needs, making accurate and timely referrals, collaborating with clinical and outreach teams, documenting interactions in the electronic record system, and meeting performance metrics and admission quotas. Required skills include strong interpersonal abilities, excellent communication, high organization and attention to detail, and proficiency with Salesforce and Google Suite/MS Office; the role requires a Bachelor's degree in health sciences, communications, psychology, social work, or related field, plus 1-2 years of relevant healthcare or patient-facing experience.
Required Qualifications
- Bachelor’s degree in health sciences, communications, psychology, social work, or related field
- 1-2 years of relevant work experience (e.g., healthcare, customer/patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach)
- Strong interpersonal, relationship-building and listening skills
- Metrics- and results-oriented mindset with experience working against concrete targets
- Met or exceeded KPIs in previous roles
- Excellent written and verbal communication skills
- Work authorized in the United States and native or bilingual English proficiency
- Proficient in Salesforce and Google Suite/MS Office
