Job Summary
Care Navigator responsible for guiding youth admissions at Charlie Health. Responsible for meeting and exceeding admission quotas, driving patient outreach and acquisition, and ensuring a seamless admissions process. Will work with clients, families, and referral sources, document interactions in the electronic record, collaborate with clinical and admissions teams, and maintain high standards of customer service and compliance. Requires a Bachelor’s degree in a related field, 1-2 years of healthcare/customer-facing experience, proficiency with Salesforce and Google Suite/MS Office, strong communication and organizational skills, and US work authorization. Notable duties include outbound referrals for non-admitted cases, building understanding of referral sources, and ensuring timely, accurately documented admissions timelines. Benefits highlighted for full-time exempt employees; compensation range $45,000–$52,500 plus performance-based bonus; state background checks may apply; location restrictions apply to several states.
Required Qualifications
- Bachelor’s degree in health sciences, communications, psychology, social work, or related field
- 1-2 years of relevant work experience (healthcare, customer/patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach)
- Proficient in Salesforce and Google Suite/MS Office
- Excellent written and verbal communication skills
- Extreme organization and attention to detail
- Work authorized in the United States and native or bilingual English proficiency
