Job Summary
Care Navigator at AWARE in Helena, MT. Responsibilities include relating positively to families needing support services, setting up and reminding appointments, providing navigation for client barriers (logistics, childcare, scheduling), addressing insurance concerns by connecting patients with resources, documenting each intervention in the EMR, developing strong working relationships with service teams, handling phones, visitors, filing, special projects, and coordinating with other agencies and service lines. Must possess sensitivity to cultural differences, strong verbal and written skills, time-management, and commitment to ethical social work practice.
