Care Navigator Case Manager for Adult Services in Brattleboro (2nd Shift) leads service planning and care coordination navigation for children’s services within mental health, manages referrals, provides primary care screening and monitoring, and serves as a primary liaison with community partners. Role requires ability to establish supportive relationships with children, youth and families, and knowledge of holistic and therapeutic case management.
