Job Summary
Answer inbound calls, emails, and text messages from patients, caregivers, families, external providers, and facilities. Schedule and maintain contact with patients via phone or email, explaining health insurance benefits, eligibility, and processing copays. Follow up after appointments to provide next steps and coordinate care. Under clinical supervision, ask screening questions to document patient safety, behavior, function, medications, and advanced care plans, as well as caregiver issues like burden and depression. Report screening results and escalate concerns to the clinical team to implement care plans. Send educational materials about Isaac and local community-based service options. Promote a compassionate environment by providing emotional support, anticipating anxieties, and answering questions within the scope of practice while maintaining HIPAA privacy.
Required Qualifications
- Bilingual Spanish
- Prior experience coordinating/navigating care for patients, caregivers, and their loved ones
- Genuine empathy
- Patience
- Exceptional time management
- Exceptional communication skills
- Ability to provide your own personal computer/laptop that meets security and telehealth standards (SOC2)
- Ability to work virtually from anywhere in the United States
- Ability to maintain participant's privacy according to HIPAA regulations
