Care Navigator - BHN (Per Diem)

The Care Navigator plays a key role in an Integrated Care team, responsible for coordinating and supporting Mental Health care within the clinic and managing referrals to other services. Key responsibilities include collecting and documenting patient data, assessing and screening patients for mental health and substance abuse disorders, providing education about treatment options, supporting medication management, and facilitating referrals to appropriate treatment programs. The role requires effective collaboration with Primary Care Providers, Behavioral Health Clinicians, and psychiatric providers, along with maintaining a registry for monitoring care outcomes.
