Job Summary
Review medical and mental health assessments for referred patients and assure necessary connections with providers. Meet individually with patients to develop work plans, remove treatment obstacles, and record progress while collaborating with clinical staff. Develop strong relationships with community providers to link patients with childcare, transportation, housing, employment, and education resources. Document all interactions in electronic records, track referrals, and maintain confidentiality per HIPAA and 42CFR. Complete required training in data programs, case management, and SOAR. Participate in planning meetings with other Care Navigators. This role supports long-term recovery for patients in Linn County through intensive case management.
Required Qualifications
- Bachelor's degree in a human services field
- One year of related work experience
- Valid driver's license
- Ability to pass an extensive background check
