Care Navigation Nurse LPN

Care Navigation Nurse LPN supports referral intake, care coordination, and initial clinical triage to ensure timely, accurate, and efficient access to hospice services. Responsibilities include retrieving, monitoring, and processing referrals from multiple sources (CarePort, electronic fax inbox, phone referrals, and other designated sources), scheduling appointments via dispatch, following up on referrals, performing direct admits to inpatient care centers as needed, taking clinical calls from patients and caregivers and escalating appropriately, notifying primary care teams of clinical issues, and participating in quality/process improvement initiatives. Requires an Ohio-licensed LPN; hospice experience strongly preferred; must communicate effectively with patients and families in distress; strong teamwork skills; proficient with EMR, phone systems, and referral/intake platforms.
