Job Summary
Care Manager/Case Manager in the Family Care Program provides comprehensive case management for program members, including assessments, care planning, coordination of services, and quality assurance. Responsibilities include managing an assigned caseload in collaboration with an RN, completing member assessments & functional screens, maintaining required documentation, and ensuring adherence to standards. Requires a Bachelor’s degree in a Human Services field, ~3 years’ experience with seniors or adults with physical/intellectual disabilities, eligibility for long-term care functional screener certification, and regular local travel. Monday–Friday, 8:00 AM–4:30 PM; full-time in Waupaca, WI with a $5,000 sign-on bonus and a benefits package. The role is with Community Care, a Wisconsin-based nonprofit dedicated to helping residents live independently with caregiver and community supports.
Required Qualifications
- Education: Bachelor’s Degree in a Human Services or closely related area required.
- A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human services degree.
- Certification: Eligibility for long-term care functional screener certification is required
- Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
