Job Summary
Care Manager coordinates and delivers case management services for individuals with developmental disability or mental illness across office, home, and community settings. Responsibilities include gathering medical/psychological/social history, evaluating needs for least restrictive environment, initiating and coordinating assessments, developing and monitoring person-centered plans, advocating on behalf of individuals, coordinating services with families and external agencies, providing referrals and staff trainings, participating in placement planning, and ensuring integrated health care linkage. Positions may serve multiple populations (Children 0-21 with SED/IDD, Adults, or Behavioral Specialist roles). Requires Bachelor’s in Human Services; licensure is preferred (RN/LBSW/LMSW/etc.); QMHP/QIDP/CMHP eligibility; experience with OBRA and various regulatory/compliance activities; veteran status preferred for veteran-specific work.
Required Qualifications
- Bachelor's Degree in Human Services Field from an accredited school
- Licensure preferred: RN, LLBSW, LBSW, LLMSW, LMSW, TLLP, LLP, LLPC, LPC
- QMHP or QIDP or CMHP where appropriate for population served
- SST licensure for staff meeting Medicaid requirements
- BCaBA for Care Manager-Behavioralist track (if applicable)
- Veteran status required for working with Veterans
- 40 hours of Children’s training per calendar year for staff working with children
- CAFAS rater training for applicable roles
- Substance license preferred
- 3+ years of experience in examination, evaluation and treatment of SED Children and families
Additional Requirements
- Must be able to perform essential duties satisfactorily
- Must meet physical requirements as described in job postings
- Valid Chauffeur’s License if transporting individuals receiving services
