Job Summary
Care Manager coordinating patient care across the continuum, performing comprehensive clinical assessments, developing patient-centered care plans, and leading multidisciplinary teams to improve quality and outcomes. Responsibilities include gathering information from medical records, communicating with healthcare providers, conducting home or site visits, coordinating all aspects of care, advocating for patient needs, and ensuring timely communication of findings. Requires documented Massachusetts RN licensure, strong case-management and interpersonal skills, and ability to work independently with minimal supervision. Preferred qualifications include a Bachelor’s degree, five years of experience in acute/subacute/home palliative care, and case-management certification; bilingual ability and valid driver's license are advantageous. Proficiency with Electronic Medical Records and Microsoft Office, plus experience in quality improvement and patient education, are highly valued.
Required Qualifications
- Massachusetts RN Licensure
- state board licensure or multi-state licensure through eNLC within three months of hire
- three years of clinical experience in an acute care setting
- Basic Life Support Certification (or obtain within three months)
