Care Manager

Assist residents with ADLs including bathing, dressing, grooming, mobility, and personal care while following Individualized Service Plans (ISP) and documenting changes. Promote safety, dignity, and independence by engaging residents through conversation and daily activities, supporting dining services, and maintaining a clean environment. Communicate effectively with families and care leaders to ensure coordinated support. In Memory Care, create enriching experiences for residents with memory loss. This role requires a High School Diploma/GED, age 18+, and CPR/First Aid certification.
