Care Manager

Review and initial Daily Logs, participate in ISP development and monthly updates, and physically check on residents throughout the shift while documenting symptoms, conditions, and changes in alertness or weight. Respond promptly to call bells and security systems, greet guests and family members, and assist with activities of daily living, continence management, and life skills enrichment. Maintain resident rooms and common areas, wash laundry and linens, serve meals, and ensure compliance with safety regulations, infection control, and dietary restrictions. Report accidents, hazardous conditions, and occupational exposures immediately, and attend daily cross-over meetings and regular training sessions.
