Job Summary
Care Manager provides hands-on care and emotional support to a designated group of residents in an assisted living/long term care and reminiscence neighborhood. Responsible for developing ISPs, documenting in daily logs, monitoring residents’ conditions, responding to call bells, promoting safety and independence, communicating with families, inviting and assisting residents with activities, ensuring infection control and compliance with Sunrise standards, and participating in team meetings and training to support quality of life for residents.
Required Qualifications
- High School diploma/GED accepted
- CPR Certificate and First Aid as required by state/provincial regulations
- Must be at least 18 years of age
