Job Summary
Care Manager (Designated Care Manager) at Sunrise Senior Living provides high-quality care to a consistent group of residents and families in assisted living/long-term care, develops and updates Individualized Service Plans, ensures safety and a home-like environment, supports resident independence through ADLs and life-enrichment activities, communicates with families, participates in safety and risk-management procedures, and collaborates with the team to deliver compassionate, person-centered care aligned with Sunrise principles and regulatory requirements.
Required Qualifications
- High School diploma/GED accepted
- CPR Certificate and First Aid as required by state/provincial regulations
- Must be at least 18 years of age
- Previous experience working with seniors preferred
