Job Summary
Care Manager responsible for providing high-quality, resident- and family-centered care in Sunrise Senior Living's assisted living/long-term care and reminiscence neighborhoods. Responsibilities include documenting in Daily Log, contributing to ISP development and updates, monitoring designated residents, observing and reporting changes in condition, responding to calls and safety needs, assisting with continence management, engaging families, supporting activities of daily living and life enrichment, maintaining infection control and safety practices, coordinating with the team, and participating in training and meetings to uphold Sunrise standards of service and resident rights.
Required Qualifications
- High School diploma/GED accepted
- CPR Certificate and First Aid as required by state/provincial regulations
- Must be at least 18 years of age
