CARE MANAGER

Coordinate care for assigned patient populations across the continuum of care, overseeing interdisciplinary activity including bedside rounds. Assess and screen patients for discharge needs, perform readmission prevention activities, and facilitate safe discharge plans aligned with patient goals. Provide referrals for continuing care and locate community resources while collaborating with management, staff, and post-discharge providers to minimize costs and service duplication. Ensure services are appropriate for specific age groups based on chronological, developmental, and psycho-social maturity.
