Job Summary
Partner with patients, families, and the healthcare team to address and advocate for social and emotional needs through psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referrals. Lead interdisciplinary internal and external teams to improve patient care by effectively utilizing and monitoring healthcare resources while facilitating desired clinical, financial, and resource outcomes. Guide daily practice according to the NASW Code of Ethics and the Swedish Mission and Vision. This combined night shift role in Issaquah, WA, requires an MSW or LCSW with a Washington Clinical Independent Social Worker license and one year of acute medical or mental healthcare experience.
Required Qualifications
- Master's Degree in Social Work from an accredited school
- Upon hire: Washington Clinical Independent Social Worker Associate License
- 1 year of experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting
- Upon hire: Washington Clinical Independent Social Worker License
