Job Summary
Care Manager Social Worker role within Monogram Health's integrated Care Team. Delivers in-home and telehealth care management, assesses social determinants of health, builds community-based relationships, educates patients and families about resources and living with chronic illness, and collaborates with the Care Team to optimize outcomes. Requires MSW and active LCSW/LMSW licensure in the posting state, BLS, and occasional travel to patient homes with telephonic/telehealth visits.
Required Qualifications
- Master’s degree in social work (MSW)
- Currently licensed as an LCSW or LMSW in the posted state
- Basic Life Support (BLS) certification
- Pass ASWB master’s or clinical exam
