Job Summary
Deliver all six NYS DOH Health Home Core Services, including comprehensive care management, care coordination, and transitional care for children and adults with complex medical and behavioral needs. Conduct required assessments to develop person-centered Plans of Care, coordinate medical, behavioral health, and social service supports, and engage members through outreach and advocacy. Collaborate with hospitals, providers, and community partners while maintaining accurate documentation in NYSDOH-approved systems. Participate in case conferences, audits, and training to meet productivity and quality benchmarks. This hybrid role requires regular travel throughout Suffolk, Nassau, and select NYC boroughs, with in-office days at Brooklyn or Dix Hills locations. Applicants must reside in New York State.
Required Qualifications
- Masters with one year of relevant experience
- Bachelor's of Arts or Science with two years of relevant experience
- License as a Registered Nurse with two years of relevant experience
- Relevant experience includes care coordination, case management, social services, behavioral health, medical care navigation, or community-based services
- Complete Children Adolescent Needs Assessment (CANS)NY training
- Be CANS‐NY certified annually
- Care Managers: minimum 70% exam score
- Supervisors: minimum 80% exam score
- Applicants must reside in New York State
- Regular travel throughout Suffolk and Nassau Counties and select boroughs of New York City
- Travel may occur multiple times per week based on program needs
- Flexibility to travel for service delivery is required
