Care Manager RN - Social Work, Multiple FTEs and Shifts Available

RN Care Manager in the acute care setting to progress patients through the continuum of care. Key services include screening for post-acute needs; transition planning; care coordination; education; community care referrals; care planning in accordance with regulatory requirements; collaboration with the continuum care team; ongoing monitoring of plan effectiveness and reassessment of the need for care management. Requires nursing degree/diploma with Oregon RN license; 2 years of acute care or community-based experience; 1 year EMR/documentation experience; 1 year in home health, mental health, substance use, hospice, utilization review, discharge planning or care management. Preferred: Bachelor's degree in nursing within 3 years of hire; Certified Case Manager. Location is on-site at Providence Portland Medical Center with multiple shifts; part-time and full-time opportunities.
