Job Summary
RN Care Manager in acute care at Providence Portland Medical Center coordinates post-acute transitions, screens for post-acute needs, plans and monitors care across the continuum, provides education to patients and families, facilitates community care referrals, develops regulatory-compliant care plans, collaborates with the care-team, and reassesses patient needs to optimize outcomes.
Required Qualifications
- Nursing degree/diploma required upon hire
- Bachelor's degree in Nursing or higher within 3 years of hire
- Oregon Registered Nurse License upon hire
- 2 years of acute care hospital or community-based experience
- 1 year of experience with electronic medical records or documentation systems
- 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
