Job Summary
RN Care Managers in the acute care setting provide care coordination and progression of patients through the continuum of care. Responsibilities include screening for post-acute needs, transition planning, care coordination, education, community care referrals, care planning per regulatory requirements, collaboration with the continuum care team, and ongoing monitoring and reassessment of care plans. Role supports both part-time and full-time schedules with multiple shifts; requires nursing degree/diploma and Oregon RN licensure, plus experience in acute care and EMR systems. Preferred qualification includes Certified Case Manager and a Bachelor’s degree in nursing.
Required Qualifications
- Nursing degree/diploma required upon hire
- Bachelor's degree in Nursing or higher within 3 years of hire
- Oregon Registered Nurse License upon hire
- 2 years of acute care hospital or community-based experience
- 1 year of experience with electronic medical records or documentation systems
- 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
Desired Qualifications
- Certified Case Manager (preferred)
- Bachelor's degree in Nursing
- Oregon Registered Nurse License
- EMR/EHR experience
