Job Summary
RN Care Manager in the acute care setting providing screening for post-acute needs, transition planning, care coordination, education, and community referrals. Responsibilities include care planning in accordance with regulatory requirements, ongoing monitoring of plan effectiveness, collaboration with the continuum care team, and reassessment of the need for care management services. Requires nursing degree/diploma upon hire, Oregon RN license, and experience with electronic medical records. Preferred qualifications include a bachelor's degree in nursing, and Certified Case Manager status. The role supports multiple shifts and part-time availability at Providence Portland Medical Center, with on-site work in Milwaukie/Portland, OR.
Required Qualifications
- Nursing degree/diploma
- Oregon RN license upon hire
- 2 years of acute care hospital or community-based experience
- 1 year of EMR or documentation systems experience
- 1 year of home health, mental health, substance use, hospice, utilization management, discharge planning or care management experience
Desired Qualifications
- Nursing degree/diploma
- Bachelor's degree in Nursing or higher within 3 years of hire
- Oregon Registered Nurse License upon hire
- 2 years of acute care hospital or community-based experience
- 1 year of experience with electronic medical records or documentation systems
- 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
- Certified Case Manager (preferred)
- Bachelor's degree in nursing upon hire (preferred)
- Proficiency with EMR systems and regulatory compliance
