Job Summary
RN Care Manager in the acute care setting provides care coordination, transition planning, education, community care referrals, and ongoing monitoring of care plans; requires Nursing degree/diploma, Oregon RN license, and within 3 years of hire a Bachelor's degree in Nursing or higher, plus experience in acute care and with electronic medical records; preferred qualifications include a Bachelor’s degree in nursing and Certified Case Manager.
Required Qualifications
- Nursing degree/diploma required upon hire
- Bachelor's degree in Nursing or higher within 3 years of hire
- Oregon Registered Nurse License upon hire
- 2 years of acute care hospital or community-based experience
- 1 year of experience with electronic medical records or documentation systems
- 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
