Job Summary
RN Care Manager in the acute care setting providing a range of care management services to progress patients through the continuum of care. Key duties include screening for post-acute needs, transition planning, care coordination, patient and caregiver education, and collaboration with the care continuum team, with ongoing monitoring and reassessment of care plans in accordance with regulatory requirements.
Required Qualifications
- Nursing degree/diploma required upon hire
- Bachelor's degree in Nursing or higher within 3 years of hire
- Oregon Registered Nurse License upon hire
- 2 years of acute care hospital or community-based experience
- 1 year of experience with electronic medical records or documentation systems
- 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
- Preferred Qualifications: Bachelor’s degree in nursing upon hire
- Certified Case Manager upon hire
