Job Summary
RN Care Managers in the acute care setting provide screening for post-acute needs, transition planning, care coordination, education, and community care referrals, with care planning in accordance with regulatory requirements; collaborate with the continuum care team and monitor plan effectiveness, supporting patients through the continuum of care at Providence Portland Medical Center. Role emphasizes nursing leadership in social-work-aligned care management, with opportunities across part-time and full-time shifts, and a comprehensive benefits package. Must hold an Oregon RN license and a nursing degree with a Bachelor’s in Nursing (or higher) within 3 years of hire; experience in acute care, EMR usage, and care-management-related areas is preferred.
Required Qualifications
- Nursing degree/diploma required upon hire.
- Bachelor's degree in Nursing or higher within 3 years of hire.
- Oregon Registered Nurse License upon hire
- 2 years of acute care hospital or community-based experience.
- 1 year of experience with electronic medical records or documentation systems.
- 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
