RN Care Manager in the acute care setting provides a range of services to progress patients through the continuum of care, including screening for post-acute needs, transition planning, care coordination, education, community care referrals, and care planning in accordance with regulatory requirements, with ongoing monitoring of plan effectiveness and reassessment of needs, collaborating with the continuum care team to support patient-focused, whole-person care.
