Job Summary
RN Care Manager in acute care setting provides post-acute needs screening, transition planning, care coordination, education, and community care referrals; collaborates with the continuum care team, with responsibilities including ongoing monitoring and reassessment of care plans. Requires nursing degree/diploma, Oregon RN license, and a Bachelor's degree in Nursing within 3 years of hire; minimum 2 years of acute care experience, plus 1 year of EMR/documentation systems experience and 1 year in home health, mental health, substance use, hospice, utilization review, discharge planning, or care management. Position offers Part-Time or Full-Time schedules at Providence Portland Medical Center with on-site work in Portland, OR.
Required Qualifications
- Nursing degree/diploma required upon hire
- Bachelor's degree in Nursing or higher within 3 years of hire
- Oregon Registered Nurse License upon hire
- 2 years of acute care hospital or community-based experience
- 1 year of experience with electronic medical records or documentation systems
- 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
