Job Summary
Seeking a Care Manager - RN to provide proactive whole person care management for pediatric patients, coordinating with families and providers to enhance healthcare outcomes.
Required Qualifications
- Registered Nurse (RN) with a valid North Carolina (NC) license or a compact RN license or eligibility to obtain a license to practice professional nursing in NC
- 3 years professional RN practice experience
Desired Qualifications
- 3-5 years of care management experience
- Bilingual Spanish
- Pediatrics experience in the outpatient, inpatient, or emergency department setting
- Value-based care experience
- Strong organizational skills
- Comfort conducting home visits or providing services in the community
