Job Summary
Assess and plan care for at-risk patients in an acute care environment, coordinating resources and managing discharge planning. Document interventions, ensure regulatory compliance, and advocate for patients to optimize healthcare resource utilization across the continuum. Requires an Associate's Degree in Nursing, a California RN license, and two years of acute care experience, with one year in care management or completion of the TIPS program. Per Diem variable shifts at Providence Little Company of Mary Torrance.
Required Qualifications
- Associate's Degree Nursing degree/diploma
- California Registered Nurse License
- 2 years Acute care experience in a Medical Surgical or Inpatient setting
- Experience in care management or utilization review in any setting
- Upon hire successful completion of TIPS program or Case Management Orientation Program
Desired Qualifications
- Bachelor's Degree Nursing or higher
- Case management certification (ACM or CCM)
