Care Manager RN Per Diem Variable

Coordinate care and needs for complex patients and their families throughout the continuum of care by planning, negotiating, and procuring services. Monitor medical records for variances to clinical and financial standards, intervening to achieve optimum outcomes for both patients and the hospital. Communicate daily with medical staff, hospital personnel, and outside resources to ensure appropriate resource utilization. This role requires an RN license and two years of acute care experience, with per diem variable 8-hour shifts at St. Joseph Hospital in Eureka, CA.
