Care Manager RN Per Diem Variable

Assess and plan care for at-risk patients in an acute care environment, coordinating care, managing resource utilization, and overseeing discharge planning. Document interventions, ensure regulatory compliance, and advocate for patients to guarantee appropriate healthcare resources are used throughout the continuum. Requires an Associate's degree in nursing, a California RN license, and two years of acute care experience in a medical surgical or inpatient setting. One year of care management or utilization review experience is needed, or successful completion of the TIPS program or Case Management Orientation Program. This role is per diem with 8-hour variable shifts at Providence Little Company of Mary Torrance.
