Care Manager RN Per Diem Variable

Plan, coordinate, and manage care for complex patients and their families throughout the continuum of care by negotiating resources and monitoring medical records for clinical and financial variances. Intervene to achieve optimum outcomes for patients and the hospital while communicating daily with medical staff, hospital personnel, and outside resources. Ensure appropriate utilization of resources according to established standards. This role involves 8-hour variable shifts at St. Joseph Hospital Eureka, CA, with a base pay range of $54.61 to $92.23. Additional compensation may include shift differentials, standby/on-call, overtime, and premiums.
