Care Manager RN Per Diem Variable

Assess and plan care for at-risk patients in an acute care environment, coordinating care, managing resource utilization, and overseeing discharge planning. Document interventions, ensure regulatory compliance, and advocate for patients to guarantee appropriate healthcare resources are used throughout the continuum. This role requires an Associate's degree in nursing, a California RN license, and two years of acute care experience, with one year in care management or utilization review preferred. Successful completion of the TIPS program or Case Management Orientation Program is required upon hire. Per diem variable shifts are available.
