Care Manager RN Per Diem Day

Coordinate case management functions for complex patients by planning, coordinating, negotiating, and managing care needs throughout the continuum. Communicate daily with medical staff, hospital personnel, and outside resources to ensure appropriate resource utilization per clinical and financial standards. Requires an RN license, acute care experience, and care management or utilization review background. Schedule includes 8-hour day shifts on a per diem basis.
