Job Summary
Plan, coordinate, and manage care for complex patients and their families throughout the continuum of care. Execute effective case management functions by procuring services, negotiating care needs, and assisting patients in utilizing appropriate health care resources. Perform discharge planning and utilization management duties within acute, sub-acute, and home health settings. Requires a California RN license, recent post-graduate case management training, and 2 years of acute care or 1 year of case management experience. This role supports a culture of patient-focused, whole-person care at a facility recognized for excellence in stroke and cardiac outcomes.
Required Qualifications
- Recent completion of a post-graduate level curriculum focusing on case management
- Current clinical experience with 2 years clinical experience in an acute care hospital setting
- 1 year current experience in an inpatient or outpatient case management setting
- California Registered Nurse License
- 2 years Case management, discharge planning, or utilization management experience in the acute, sub-acute, home health setting or managed care environment
- 3 years Acute Care Experience
